Peralta, Rafael C., of Hollywood, entered into eternal rest peacefully surrounded by his family on Saturday, July 19, 2014, at the age of 48. He was born on September 21, 1965, in Queens, New York, to the late Jose Raul and Melania Mercedes Peralta. Rafael is survived by his devoted wife, Agueda Peralta and his two loving children, Maria Peralta and Rafael Peralta II. Visitation will take place on Tuesday, July 22, 2014, from 6-8 p.m. at Boyd-Panciera Hollywood Boulevard Chapel. Interment will be held at Mt. St. Mary Cemetery in Flushing, NY.
