Jack N. Theodore, a long time resident of Hollywood, passed away peacefully at home surrounded by his family on Wednesday, November 11, 2015, at the age of 75. Jack was born on July 19, 1940, in Wilmington, North Carolina. He was the son of the late John Theodore and Bertha Walton. He is survived by his son, Mark Theodore and daughter, Sunday Theodore.
